Staff may provide transportation for Career Explorations and interviews.</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">While on assessment or career explorations, all individuals are covered by <B>SELECTIONS  </B>liability insurance and <B>Worker’s Compensation</B>.  During  employment or  volunteer placement, Worker’s Compensation is covered by the employer. <B>SELECTIONS  </B>maintains liability on all its participants.  Staff members carry appropriate vehicle  insurance for transporting clients when required.</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">In all work situations the individual is expected to c<B>omply with company policies</B> and procedures as outlined by their employer.</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">Individuals must <B>take medication independently</B>.  Staff are not permitted to administer medication of any type.</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">Individuals work with a Coordinator in establishing their <B>Individual Career Plan</B>.  Discussions with parents/guardians and/or residential staff regarding the individual’s performance occur on an ongoing basis and as required/requested.  The Coordinator will act as the liaison to the employer.</div> <bR> <div> <B><IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">SELECTIONS </B>likes to ensure that there is <B>ongoing communication </B>with parents and/or   guardians.  This is done through telephone calls, letters and meetings.</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">Holidays: as the individual works for a private employer, holidays are subject to their approval. Please discuss<B> holiday requests</B> with the Coordinator and the employer</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">Appointments: appropriate notice should be given regarding <B>doctor’s appointments or days off </B>required - subject to employer’s approval and notification of the Coordinator.</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">Individuals accepted into the program <B>must have all necessary forms completed</B> and meet Service Eligibility Criteria.</div> <bR> <div> <B><IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">Individuals under guardianship must have guardian approval.</B>  If the individual is under  guardianship, a copy of the Guardianship Order is required for the individual file.</div> <bR> <div> <IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">On occasion <B>AISH </B>confirms employment with <B>SELECTIONS</B>.  You are therefore required to submit your pay cheque stubs to the AISH office on a monthly basis.</div> <bR> <div> <B><IMG BORDER="0" SRC="Symb075c00b700f000000000.png" HEIGHT="16" WIDTH="24" ALIGN="bottom" HSPACE="0" VSPACE="0">This information applies to all clients</B> participating in the <B>SELECTIONS </B>program and may need to be referred to throughout your involvement.  As changes occur updated correspondence will be provided.</div> <bR> <DIV ALIGN="LEFT"></DIV> <div> <I><B> </B></I><A HREF="id24.htm" TARGET="_top" TITLE="Intake"><U><I><B>Next Page</B></I></U></A><I><B>   </B></I></div> <div>                                                                                                                                                          </div> <bR> <bR> </td> </tr></table></body>
